Rehabilitation Success Story - Ms. Gibson

We are pleased to share another wonderful rehab success story from Ms. Gibson!

Admitting Diagnosis: ICD-10-CM Diagnosis Code I26.99 - Other pulmonary embolisms without acute cor pulmonale LOS: 25 days Discharge Location: Private Residence with HHA & Rehab services

Success Story: Ms. Gibson was admitted to our facility after a hospital stay that left her deconditioned and unable to return home at her PLOF. At 90 years old, she was still very active and was Modified Independent with all ADLs. Her goal was to return to her home with her daughter. At the time of evaluation, Ms. Gibson displayed muscle weakness and fatigue with activities, decreased gait tolerance and shuffled gait pattern, increased assistance with ADLs, and verbalized that she was beginning to forget things she usually wouldn’t. After almost 4 weeks of therapy, Ms. Gibson has made excellent progress. She has met the goals set in her plan of care, placing her at her highest level of function. Ms. Gibson has graduated from therapy Independently using a rollator walker with a safe technique, can bathe and dress herself, and was taught skills to continue working on her cognition. Ms. Gibson will be returning home with her daughter at discharge.
